What Services Are Provided?

Assessment:  A staff member from the Case Coordination Unit (CCU) makes the first visit to the home. This visit is necessary to formulate a service plan. The CU refers the client to Tri-County Opportunities Council who is responsible for client care.

 

Homemakers:  Trained, responsible personnel offer support to clients, which includes meal preparation, personal care, tasks of a non-medical nature, light housekeeping, laundry, shopping and transportation to local medical facilities.

 

Who Is Eligible?

Clients interested in receiving services should:

· Live in Carroll, Lee, Ogle or Whiteside counties in Illinois

· Be 60 years of age or older

· Be a US citizen or legal alien

 

All services are available on a private pay basis for clients not meeting the income guidelines—regardless of age.

 

How Are Services Obtained?

Persons may inquire about services by contacting Tri-County Opportunities Council in Rock Falls. Clients are also referred for services by:

· Case coordination units

· Hospital social workers

· Doctors

· Nurses

· Health Departments

· Information and referral services

· Clergy

· Friends

· Clients

 

The Homemaker staff at Tri-County Opportunities is available for referrals Monday through Friday from 8:00 a.m. - 4:30 p.m. Services by Homemakers are provided Monday through Friday  from 7:00 a.m. - 8:00 p.m., Saturday from 7:00 a.m. - 6:00 p.m. and Sunday from 8:00 a.m. – 5:00 p.m.
